Students in Pre-K and Kindergarten will be administered over the counter medication with written parental permission.\u00a0 The permission and medications available are listed on the back of the emergency card.\u00a0 Tylenol will only be given after all palliative measures have been tried.\u00a0 If a student requests Tylenol more than three times in a month, a parent\/guardian will be notified and requested to seek further medical consultation.<\/p>\n
Students are not allowed to carry their own medication to and from school<\/strong>.\u00a0 A parent\/guardian or a responsible adult must bring the medication in its original pharmacy labeled container to your child\u2019s nurse. *(The exception for students not carrying medications is Epi-pens and inhalers (if parent and nurse agree)*<\/p>\n If your child needs medications during the school day such as; daily medication, inhalers, epi pen, insulin, please contact the school nurse for the following forms:<\/p>\n 1.\u00a0\u00a0\u00a0\u00a0\u00a0 Signed consent by parent\/guardian to administer medicine during school hours.<\/p>\n 2.\u00a0\u00a0\u00a0\u00a0\u00a0 Signed medication order by student\u2019s physician\/nurse practitioner<\/p>\n 3.\u00a0\u00a0\u00a0\u00a0\u00a0 Field trip permission form<\/p>\n 4.\u00a0\u00a0\u00a0\u00a0\u00a0 Asthma, epi-pen forms if indicated<\/p>\n
