Massachusetts Department of Public Health regulation 105 CMR 220 mandates all students meet the following requirements for entering grade 7:
  • 2 doses of varicella vaccine (unless physician signed proof of chicken pox disease)
  • 1 dose of Tdap
  • 3 doses of Hepatitis B

 

The following immunizations are recommended by for students entering grade 7:
  • Meningococcal
  • Influenza (annually)
  • Human Papilloma Virus
  • 2 doses of MMR (measles, mumps, rubella)
The immunization record must include the month, day and year the immunization was administered and must be signed by the physician.

Exemptions

If there are medical reasons why your child has not been immunized, a signed certificate must be obtained from your physician. 
 
If immunizations conflict with sincere religious beliefs, a parent/guardian must write a note stating so.
 
Children who are not fully immunized may be excluded from school from day 10 through day 21 following exposure if there is a break of disease (i.e., whooping cough, chicken pox, etc.).
 
 
Please remember that it is the parent/guardian responsibility to obtain the necessary immunizations for school.  Failure to comply with this mandate
can result in your child being excluded until the requirements are met.
