Physical Examination Requirements
In accordance with Massachusetts laws and regulations (M.G.L. c 71, s 57 and 105 CMR 200.000), physical examinations are required within one year prior to entrance to school or within 30 days after school entry, and at intervals of either three or four years thereafter. Physical examinations must be completed and signed by a licensed healthcare provider, and dated within one calendar year at time of submission.
All students are required to have proof of physical examination upon enrollment, as well as in kindergarten and grades 4, 7, and 10. It is also encouraged that physical exams be submitted to the school nurse every year, so that students’ health information can be kept current.
Immunization Requirements for School
In situations when one or more cases of a vaccine-preventable or any other communicable disease are present in a school, all susceptible, including those with medical or religious exemptions, are subject to exclusion as described in the Department of Public Health’s reportable Diseases and Isolation and Quarantine
